Use this form if you have decided to appoint Veteran Advocacy Group as your Accredited Representative. This form is secure for your protection. Upon completion of your intake, you will be automatically entered into our system and we can begin the work to secure the benefits you deserve, starting with a VA form 21-22a appointing us as your representative and a fee agreement if necessary.
